Axial postcontrast T1 (A) shows nodular enhancement in the right postcentral gyrus and subcortical white matter. There is some abnormal leptomeningeal enhancement extending into the right central sulcus (arrow). Axial T2 FLAIR (B) shows corresponding hyperintensity in the regions of enhancement. There is minimal to no vasogenic edema or mass effect on the surrounding tissue. Gradient-echo imaging (C) demonstrates no drop in signal to suggest hemorrhage or calcification. DWI (D) and ADC map (E) do not show restricted diffusion. Following biopsy, a specimen stained with hematoxylin and eosin at 200X magnification (F) shows vascularized neuronal pseudopapillae lined by smaller cuboidal cells.
