Upon in-hospital admission, she underwent a chest x-ray and abdominal ultrasound with no relevant findings. Blood analysis showed microcytic anemia, normal leukocytes count, and a PCR of 7.35 mg/dl. She had a moderately increased ADA 19, with normal IGRA, negative antinuclear antibodies, and normal Igs profile, without other relevant findings. Serologies (VHA, VDRL, CMV, Rose Bengal test, Coxiella), hemoculture, and coproculture were unremarkable. An occult primary tumor was suspected, so she performed a thoraco-abdominal-pelvic CT.
