Skull Base CSF Leaks: Potential Underlying Pathophysiology and Evaluation of Brain MR Imaging Findings Associated with Spontaneous Intracranial Hypotension
Ian T. Mark, Jeremy Cutsforth-Gregory, Patrick Luetmer, Ajay A. Madhavan, Michael Oien, Paul Farnsworth, Girish Bathla, Steve Messina, Michael Link and Jamie Van Gompel
    American Journal of Neuroradiology October 2024, 45 (10) 1593-1596; DOI: https://doi.org/10.3174/ajnr.A8333

The purpose of this study was to use a validated brain MRI scoring system (Bern score) to evaluate patients with confirmed skull base CSF leaks for findings associated with SIH. The authors found that patients with skull base CSF leaks did not have brain MRI findings that are associated with SIH and likely have distinct pathophysiology from spinal CSF leaks.

    American Journal of Neuroradiology January 2025, 46 (1) 24-32; DOI: https://doi.org/10.3174/ajnr.A8469

This review discusses the 3 key MRI sequences for ARIA monitoring and standardized imaging protocols and provides imaging recommendations for 3 key patient scenarios. All patients on anti-amyloid immunotherapy should have T2* gradient-recalled echo (to evaluate for ARIA-H), 2D or 3D T2 FLAIR (to evaluate for ARIA-E), and DWI (to differentiate ARIA-E from acute ischemia). Patient imaging scenarios are 1) baseline dementia diagnosis/treatment enrollment evaluation, 2) asymptomatic ARIA monitoring, and 3) evaluation of the symptomatic patient on anti-amyloid immunotherapy.

    American Journal of Neuroradiology January 2025, 46 (1) 129-135; DOI: https://doi.org/10.3174/ajnr.A8431

Distinguishing radiation necrosis from tumor progression after radiation therapy for brain metastases is challenging on conventional MRI. This study demonstrated higher normalized contrast-enhanced T1 and T2 FLAIR signal intensity for RN. Contrast-enhanced T2 FLAIR signal intensity distinguished RN and TP with an AUC similar to that of DSC perfusion.

    American Journal of Neuroradiology August 2024, 45 (8) 1076-1080; DOI: https://doi.org/10.3174/ajnr.A8269

The authors compared low-field portable and standard MR images of ventricular volume in the same pediatric patient. They showed high correlation in the estimates of ventricular volume between the 2 types of MR imaging, without systemic bias.

Comprehensive Review of Inner Ear Anatomy on Photon-Counting CT

    American Journal of Neuroradiology May 2025, 46 (5) 862-867; DOI: https://doi.org/10.3174/ajnr.A8410

Compared to traditional energy-integrating CT detectors, photon-counting CT allows greater spatial and contrast resolution of small channels containing nerves, arteries, and normal anatomy such as the cochlear cleft and cochlear and vestibular aqueducts. This review article updates the existing literature by redefining these structures in greater resolution.

    American Journal of Neuroradiology September 2024, 45 (9) 1185-1193; DOI: https://doi.org/10.3174/ajnr.A8214

This review explores how the International League Against Epilepsy (ILAE) subtypes of hippocampal sclerosis correlate with MRI findings. Hippocampal anatomy is reviewed in detail. The pathophysiology of hippocampal sclerosis is discussed. A radiologic classification scheme is proposed that aligns with the ILAE pathology classification, aiming to improve clinical communication and decision-making.
