Subconjunctival Fat Prolapse
- Subconjunctival fat prolapse is an acquired herniation of intraconal fat due to weakening of the Tenon capsule by the aging process, trauma, or surgery.
- Usually seen in elderly obese men, with a mean age of 65–72 years, typically as bilateral (less often unilateral) soft, yellowish mass with a convex anterior margin and superficial fine blood vessels.
- Key Diagnostic Features: Herniation of intraconal fat into the extraconal compartment. Typically this herniated fat is seen in the epibulbar temporal region bilaterally.
- DDx:
- Dermolipoma
- Epidermoid
- Rx: Surgery for cosmetic purposes or if the prolapse causes discomfort
